2025 Registration Form

 For reference, here are two items that will be filled out within the registration form:

 

Student medication forms

Please use these if you will need to send medications along with your camper. Plan ahead! For prescription medication, this form will need to be signed by a physician. At no time may any camper, regardless of age, carry medication of any kind, prescription or otherwise--this includes things as basic as cough drops!​

FEES

Full Day Camp:

  • Registration fee - $40/family

  • Weekly fee, per child

    • Regular camp day, 8:30-4:30 - $310/wk​

    • Extended camp day, 7:30-5:30 - $360/wk

PAYMENTS

All payments for Evergreen Summer Adventures will be through Evergreen's payment portal, MySchoolBucks. The registration fee is due upon registration, and is paid in Evergreen's SchoolBucks Store. The registration fee can be paid for as a guest, but weekly fees bust be paid through a parent account.
